To deliver person-centred, holistic support to lonely and isolated older people living in Middlesbrough. This includes providing non-time limited support via the telephone, in people’s homes and in the community.

The project will identify, engage, and support the loneliest and most isolated older people (aged fifty and over). The service aims to:

  • Build trusting relationships with older people using befriending and person-centred approaches
  • Support people to overcome the main difficulties in their lives which are impacting on their health and wellbeing
  • Support people to move towards a point where they feel able to re-connect with people and/or activities in their local community (where appropriate and in line with the person’s own goals)
  • Improve people’s feelings of companionship and social inclusion
